Wine Pre-Order Form

We've tried to make the process as painless as possible for everyone - all you need to do is:

1) Make sure your name and the name of your party (or table organiser) is at the top of the form so we know who to deliver the wine to! We'll also need your address for the receipt.

2) Payment is accepted by cash or cheque; if paying by cheque, please make it payable to CHALGROVE CHARITY BALL

(NB - the bar on the night is cash only, so this is a good opportunity if you want to pay by cheque!)

3) Send your completed form with your full payment to:

4) A receipt will be issued for all pre-booked wine orders. Please make sure that you keep this safe, as you will be required to present it to the bar on the night.
